- Choose the state that issued your driver's license from the License State dropdown.
- Provide your date of birth as it appears on your birth certificate in the Date of Birth field.
- Enter the date when the violation occurred in the Violation Date field.
- This eligibility checker is not a complete eligibility check. If after entering the requested information it shows you are eligible contact a defensive driving school for a full eligibility check.
